  • Any other names that may be attached to the mineral rights, and what their relation is to you (examples: parent, spouse, sibling, trust, or business)

  • Are your minerals currently leased or unleased?

    • If your minerals are currently leased, please try to include the following:

      • Name of company whom the lease was signed with

      • Date of initial signed lease agreement

      • Lease term (generally 3 or 5 years)

        • If lease was extended, please include signing date and agreed upon lease term.

        • If lease was not extended or terminated, please include date of ended lease term.

      • Percentage of royalties received (if possible, please include whether they are net or gross)
